patients with irregular corneas, high refractive error, keratoconus, corneal ectasia, and corneal deformities secondary to surgery or trauma.

These patients may benefit from a wide variety of specialty lenses such as sclerals, hybrid contacts, reverse geometry lenses, gas permeable lenses, and/or prosthetic tinted contacts. All of these lenses are made-to-order based on the patient’s corneal mapping, prescription, and lifestyle needs.

SynergEyes, Inc. is a specialty contact lens company that offers a multitude of hybrid contact lens designs for astigmatism, presbyopia, and irregular corneas, including UltraHealth. SynergEyes strives to create a straightforward fitting process via empirical ordering of Duette hybrid contact lenses through an online calculator and through consultants.

As for sclerals, practitioners may find success with Atlantis sclerals from X-Cel Specialty Contacts, Jupiter sclerals from Essilor, or Zenlens from Alden Optical due to their ease of fit. Since these specialty lenses are not available at big box stores or online retailers, these patients will rely on your skill and expertise and will return to your office on an annual basis.

Specialty contact lens fittings are time consuming and therefore warrant a premium fitting fee. Don’t be shy to charge appropriately for extra chair time and staff time. These patients will require multiple visits and hand-holding to a certain degree for optimal success. Having a well-trained and knowledgeable staff will greatly improve clinic flow.

Creating a good relationship with contact lens manufacturers and their consultants is also essential, especially for new and inexperienced practitioners. With trial and error, diagnostic fittings will become straightforward and routine. Consultants greatly aid in making adjustments and design changes based on your fitting assessments.

Keep in mind that most vision plans and medical insurance plans reimburse not only for the contact lens purchase, but also for the fitting fee and any necessary special testing performed such as topography, pachymetry, and specular microscopy. In addition, each subsequent follow-up visit can also be billed to insurance.

With the potential for high revenue and patient retention, it is a surprise that more practitioners do not fit specialty contact lenses, especially with consultants and CE courses readily available.
