This three-prong approach to keeping patients happy and healthy in their lenses will prevent them from dropping out.

Contact lens technology has advanced so much over the years, yet lens comfort continues to be the top reason patients””especially those with dry eye””stop wearing their lenses. Any approach to contact lens success then should focus on three areas: finding the best lenses and materials for each patient, helping them keep their ocular surface healthy, and teaching them proper lens care.

THE RIGHT STUFF
Bausch + Lomb’ ULTRA contact lenses (samfilcon A) with MoistureSeal’ technology provide superior comfort and vision throughout the day compared with the leading silicone hydrogel lenses, according to the company. Thanks to the MoistureSeal technology, the lens maintains 95% of its moisture for up to 16 hours. In a recent Bausch + Lomb performance study, 95% of patients agreed that ULTRA lenses provide exceptional clarity and comfort, helping to make the lens the top refit brand in the country.

Also from Bausch + Lomb, Biotrue ONEday’ (nesofilcon A) contact lenses start each day with 78% water””the same water content as the cornea””and maintain more than 98% moisture throughout the day for up to 16 hours. Hydrophilic polymer molecules are bound to the molecules of a Surface Active Macromer, Poloxamer 407, which increases in concentration at the surface of the lens and creates a dehydration barrier that mimics the lipid layer of the tear film.

Alcon Laboratories, Inc.’s Dailies Total 1′ (delefilcon A) are the first and only water-gradient lenses. The highly lubricous outer surface is composed of an ultrasoft surface gel that makes up about 10% of the lens thickness and is composed of a non-silicone hydrophilic polymer network with extended surface polymer chains. The water gradient occurs between the silicone-hydrogel core (33% water) and the surface where the water content rapidly increases to more than 80%. As the material shifts from a silicone-rich core material to an essentially silicone-free hydrophilic surface gel, the lens surface mimics the natural hydrophilic ocular surface where the water content approaches 100% at the very outer surface, similar to the precorneal tear film. Dailies Total 1 lenses have a very low surface modulus that enhances comfort and a moderate modulus silicone hydrogel core that facilitates ease of handling. With a Dk/t of 156 (at -3.00 D), they offer the highest oxygen transmissibility of any daily disposable.

While silicone hydrogels have increased tremendously in popularity, conventional hydrogels still have a large patient base. These low modulus lenses have a high water content, but employ technology to hold water molecules in the lenses. CooperVision Inc.’s Proclear’ family of lenses uses PC Technologyâ„¢, which keeps lenses healthy, moist, and comfortable throughout the entire day. The Proclear lenses are the only ones that have FDA-cleared indication that they “may provide improved comfort for contact lens wearers who experience mild discomfort or symptoms relating to dryness during lens wear.”

HEALTHY HABITS
Meibomitis, meibomian gland dysfunction (MGD), blepharitis, and poor tear composition are detrimental to comfort success so encourage good lid and lash hygiene and stress the importance of keeping a healthy ocular surface. If patients listen to your advice, they’ll avoid uncomfortable complications that can result in reduced hours of comfortable lens wear.

Patients generally do well controlling MGD and blepharitis by regularly using a product such as OCuSOFT’ Lid Scrub’. This surfactant-based product is used to clean the eyelids and eyelashes, removing debris and excess oils from the lid margins and diminishing bacteria while also reducing staph blepharitis.

A newer Rx eyelid cleanser, NovaBay’ Pharmaceuticals, Inc.’s Avenovaâ„¢ with Neutroxâ„¢ (a non-antibiotic antimicrobial) is the only lid hygiene prescription product designed for inclusion in all daily regimens for MGD dry eye and blepharitis. When tested in-solution, Avenova with Neutrox demonstrates a fast-acting broad spectrum of activity. Neutrox is pure hypochlorous acid, which is a naturally occurring compound that provides an oxidative burst produced by neutrophils and monocytes.

TAKING CARE OF BUSINESS
The third piece of the puzzle is making sure your patients know the best way to care for their lenses. Don’t be shy about recommending specific cleaning solutions, as certain products perform better and are more compatible with different lens materials. And be sure patients understand the steps involved in cleaning their lenses, including the need to rub them to remove debris and deposits.

Bausch + Lomb’s Biotrue’ multipurpose solution matches the pH of healthy tears and contains the same lubricant””hyaluronan””that’s naturally found in the eye. It boasts both a dual disinfection system to kill bacteria and a protein management system that maintains the anti-microbial activity of native tear proteins. Biotrue helps keep lenses moist for up to 20 hours.

CLEAR CARE’ Cleaning and Disinfection Solution from Alcon utilizes Triple Action Cleaning that deep cleans and loosens dirt using a surfactant while bubbles enhance protein removal. CLEAR CARE uses hydrogen peroxide to kill bacteria and germs that cause eye infections. Patients simply rinse their lenses, fill the special lens case with CLEAR CARE solution, and let their lenses soak for six hours. The unique lens case then neutralizes the solution to become a gentle saline solution, much like tears with no added preservatives. CLEAR CARE now has an improved lens case with baskets that differ in color to help users more easily differentiate between their left and right lenses before and after cleaning. OO
